Referring a patient for private‑duty nursing services is as easy as 1, 2, 3.

STEP 1: Gather Information

You may use my referral form or a form of your own. Please include at a minimum, the patient's name & contact information.

STEP 2: Contact Jasson Felcher, LVN

Fax your referral form & orders to: (209) 682-1144
You may also call or Text Jasson at‪ (209) 418-7615‬ with a notification that your referral is ready. Please include the facility, referring provider's name & call back number.

STEP 3: Expect a Timely Response

You can expect a response within 24-48 hours. For urgent cases, you may send a TEXT to ‪(209) 418-7615‬ to ensure prompt attention is given to your referral.
